Community Partnerships has over 15 years experience coordinating community-based mental health services for individuals in Dane County living with mental illness. Our Service Facilitators specialize in support and care that is strengths-based, trauma-informed, and participant-led.
Community Partnerships offers CCS Service Facilitation and some additional services from the CCS Service Array for two age groups:
- Youth (age 4-17 years old)
- Adults (age 18-30 years old)

Youth CCS Services
Our youth CCS services include:
- Help with finding a therapist or other mental health professional to work with your child or family.
- Help with arranging, monitoring, or directly assisting with psychiatric services and medication management.
- Crisis planning and support.
- Help with finding a doctor or physician to meet your child’s physical health needs.
- Help with finding and keeping affordable housing.
- Help with figuring out what benefits (SSDI, Section-8, food stamps) you are eligible for and how to access those benefits.
Adult CCS Services
Our adult CCS services include:
- Help with arranging, monitoring, and directly assisting with psychiatric services and medication management.
- Help finding a therapist or other mental health services.
- Help finding drug and alcohol services.
- Crisis planning and support.
- Help with figuring out what benefits (SSDI, Section-8, food stamps) you are eligible for and how to access those benefits.
- Help with finding and keeping an apartment or living arrangement.
- Help with finding and keeping a job.
- Help with finding a doctor or physician to meet your physical health needs.
