Provider Network

The Children Come First (CCF) program operates a provider network comprised of both traditional and non-traditional services.  When youth are enrolled in CCF, their Coordinators and Clinical Specialists are responsible for coordinating and authorizing services.

If you are a parent and would like more information on our Provider Network or a specific service, please contact your CCF Coordinator or Clinical Specialist.

Provider Change Form: Complete this form if you need to add, remove, or edit any clinician’s information for your organization.  

Prior Authorization Form Instructions

Prior Authorization Form: Clinicians/personnel providing a clinical service to any CCF client need to complete this form every 90 days and submit it to Community Partnerships.
