Sobre los Servicios Comunitarios Integrales en RISE

Los Servicios Comunitarios Integrales (CCS, por sus siglas en inglés) son un programa de Asistencia Médica administrado por el Departamento de Servicios Humanos del Condado de Dane.
CCS ofrece una variedad flexible de servicios individualizados de rehabilitación psicosocial basados en la comunidad para personas de todas las edades que enfrentan problemas de salud mental y/o abuso de sustancias.

RISE ofrece Facilitación de Servicios CCS y servicios adicionales para personas de 6 años en adelante.

 

Facilitacion de Servicios CCS

 

RISE ofrece Facilitación de Servicios CCS y servicios adicionales para personas de 6 años en adelante, como desarrollo de habilidades y psicoeducación para quienes reciben facilitación de servicios a través de nuestra agencia.
La facilitación de servicios incluye colaborar con el participante inscrito y su familia, ayudar a guiar al equipo coordinando servicios y estableciendo enlaces adecuados para apoyar a la persona o familia en el cumplimiento de sus metas.

Como Hacer un Referido

Todas las remisiones para la Facilitación de Servicios CCS deben pasar por el Departamento de Servicios Humanos del Condado de Dane.

  • ¿Ya estás inscrito en CCS? Si estás interesado en los servicios de RISE, como Psicoterapia, Psicoeducación Individual/Familiar o Desarrollo de Habilidades Individuales, habla con tu facilitador de servicios sobre cómo enviar una remisión a través de nuestro formulario en línea.

 

Desarrollo de Habilidades Individuales (ISD):

Los servicios de ISD pueden incluir el desarrollo de habilidades en comunicación, relaciones interpersonales, resolución de problemas y otras necesidades específicas identificadas. También pueden abordar habilidades para la vida diaria, como cuidado personal, tareas del hogar, manejo financiero, acceso a recursos y servicios comunitarios, y otras necesidades diarias incluidas en el plan de recuperación del participante.

Psicoterapia:

Los terapeutas de psicoterapia trabajan desde un enfoque basado en el trauma y las fortalezas. Cada terapeuta está capacitado en diversas modalidades clínicas, como Terapia Cognitivo-Conductual (CBT), Terapia Dialéctico-Conductual (DBT), mindfulness, terapia basada en el apego, terapia basada en el juego, Terapia Cognitivo-Conductual para el Trauma (TF-CBT), EMDR, entre otras.

Psicoeducación Individual y Familiar (IFP):

Los servicios de IFP están diseñados para apoyar a individuos y sus familias en el manejo de problemas de salud mental y abuso de sustancias. Los proveedores de psicoeducación pueden ayudar a comprender la salud mental y los síntomas, aprender estrategias de afrontamiento, mejorar relaciones interpersonales y manejar factores de estrés diarios para promover la recuperación y estabilidad.

Felicia Jenkins (She/Her)

CCS Service Facilitator

Services: Service Facilitation & ISD

Felicia is pursuing a master’s degree in social work though Cappella University in MN and has received her bachelor’s degree in psychology and social science from National-Louis University, in Chicago. Felicia has been working in mental health for over 20 years. Felicia has provided services to the clients who identifies as apart of the developmentally disabled (DD) population as well as with a mental health diagnosis. Felicia currently provides service facilitation for both adolescents and adults and strives to support her clients with the support they need that will help them not only stay in the community, but to also be successful while doing so. Felicia believes that the client is the expert of themselves!  Felicia appreciates the opportunity to walk alongside clients, connecting them with the appropriate support, help with empowering them, cultivates a more optimistic outlook and can increase motivation for change and success in reaching their goals.

Felicia enjoys spending time with her family as family is important to her. Felicia also enjoys traveling and learning from different cultures when traveling internationally.

 

Becca Smith (She/Her)

CCS Service Facilitator

Services: Service Facilitation, IFP & ISD

I grew up moving around and found a love for travel and adventure from it. I graduated from Purdue University with my master’s in psychology. I have experience working with children and adolescents in different stages of their life starting as early as preschool up through college. I love movies, reading, travel, and spending time with everyone in my life. My hope is to help participants grow and meet their goals and feel supported as they do so. My favorite part about RISE is that the families voices are always heard and appreciated and that it’s filled with individuals who deeply care about the people they serve.

A few areas of my professional interests include trauma informed care, education, harm reduction, and building self-esteem so that individuals can rise to be the best version of themselves! Through service facilitation and independent/family psychoeducation I take my passion for music, the outdoors, games, creativity, and patience to build trust and confidence so that my participants can flourish!

 

 

 

Kate David (She/Her)

CCS Service Facilitator

Services: Service Facilitation & ISD

Kate is very interested in working with teens and folks who are transitioning into adulthood age. Kate prioritizes creating a safe space and a trusting relationship with those she works alongside. She provides service facilitation and individual skill development and enhancement. As someone who identifies as a queer woman, Kate enjoys supporting folks who identify with the LGBTQIA+ community and their own journey.

Kate is currently pursuing a master’s in social work at UW-Madison. In her free time, Kate enjoys hiking, being in nature, and spending time with friends.

 

 

Karissa Smaglick (She/Her)

CCS Service Facilitator

Services: Service Facilitation & ISD

Karissa looks to support adolescents and young adults with challenges around depression, anxiety, self-esteem, difficulty concentrating, identity issues, suicidal ideation, and self-harming. Karissa aims to provide support using a strength-based and participant-led approach combined with art techniques for Individual Skill Development. She hopes to use art to practice building coping skills, mindfulness, distress tolerance, self-patience, and creativity.  Karissa is a transracial Chinese adoptee (transracial meaning one being raised by a family/community that differs from an individual’s culture of origin) and identifies with the LGBTQ+ community.

She is passionate about exploring the intersection of identity and mental health. Outside of CCS, Karissa enjoys creating art, cooking, gaming, and tending to her indoor plants!

 

 

Shalynn Krueger (She/Her)

CCS Program Supervisor

Services: Service Facilitation, Psychotherapy & IFP

Shalynn enjoys supporting adolescents, adults, and families with a range of support needs, including anxiety, depression, caregiving problems, postpartum, life transitions and trauma. She believes in being strengths based and the power of empathy. Shalynn uses a person-centered approach that pulls from a variety of models such Cognitive Behavioral Therapy, mindfulness, DBT and Motivational Interviewing . She is passionate about providing a safe place for everyone to feel heard and walk alongside folks in their recovery.

She enjoys fall activities, listening to music, and spending time with friends and family. Shalynn has a young child and identifies as a white, cis woman.
