About Transition to Independence Program
The Transition to Independence Program (TIP) provides community-based care coordination and support for mental health recovery for young adults, ages 18-30 years old. TIP is designed to be a transitional program, meaning that some participants work with TIP for longer periods of time, while others meet their recovery goals in less time. TIP services provide an “at your service approach” to care coordination, goal setting, and social support.
